Independent desktop and clinical reviews for cases that do not meet the threshold for statutory review processes but where there is still meaningful scope for learning and improvement.
When a Desktop Review Is Right
Not every case that warrants review meets the criteria for a statutory process. Desktop reviews offer organisations a proportionate way to examine practice, identify learning and drive improvement without the full framework of a SAR, DARDR or CSPR.
Beyond the File is open to commissions to conduct independent desktop and clinical reviews, applying the same level of analytical rigour as we would for statutory reviews. We tailor our methodology and terms of reference to meet the needs of your agency and the aims of the review.
Our Experience
Examples of desktop reviews we have been involved in include:
- A near-miss domestic homicide involving a patient with a forensic history and a severe mental illness
- The management of a suicidal patient's leave under the Mental Health Act
In each case, we worked closely with the commissioning organisation to define clear terms of reference, conducted a thorough analysis of records and practice, and produced actionable recommendations designed to strengthen systems and improve outcomes.
